All students interested in participating in high school sports must have physical examination (and the corresponding completed forms) on file. On Saturday October 8, 2011, students interested in completing the process for Spring 2012 sports may do so $20 fee. If your athlete needs a physical, he/she should come between the hours of 10:00 AM and 2:00 PM - in the upper level gymnasium and health rooms at Milton High School.
They must have a Participation Physical form partially completed along with a check in the amount of $20.00 made out to their respective booster club. (Those doing so for Soccer should make checks payable to "Milton Soccer Booster Club.")
Two of the doctors present will be from Doctor’s Express. They will also have additional staff here offering flu shots to anyone who would like them for a fee of $25.00. This is available to students and parents.
